    Slovak Archives

    You can request genealogical research for documentation older than 30 years by filling out the applications below. The first one is a request for a record of the Slovak ancestors in a so-called running account. This application allows also requesting duplicates of their vital records, such as the birth certificate. The second one is a request for issuing individual vital statistic records, such as birth, marriage, or death certificates. You can fill out the applications and send them directly to the following address:

    When sending the applications, please consider also including copies of any supplemental documentation or records you may have about your ancestors. That may enhance the success of the research in the archives. Once the research is completed, you will receive a notification directly from the archive with information on how to send the payment for the service. Upon receipt of the payment, the archive will send the reports or records directly to your address.
