Citizen of the Slovak Republic has duty to notify loss of the passport, ID card or Driving license to the Embassy.
Notification should be make by fulfilled form –
With notification, it is necessary to submit scan of passport, ID card, Driving license or any other document proving the identity of citizen.
We recommend notifying the loss or theft of the passport, ID card or driving license to closer police officer/department and apply for confirmation of the notification, if that is possible.
In case that your debit/credit cards, bank checks, etc. were stole too, we recommend to contact the issuer of cards or checks.
