Electronic filling of visa application


The Embassy of Slovakia in Jakarta would like to inform that with effect from 25 April 2012, an application for electronic filling the visa application associated with the reservation of an appointment for submission of applications at the Embassy is launched.

Electronic visa application is available at https://ezov.mzv.sk

The visa applicant shall complete an electronic visa application – by entering electronically via the application all data according to the visa application (personal data, information about their trip etc.).
After confirmation of the application system will generate a completed visa application form (pdf format) with an assigned code of the application.
The applicant will print the form and sign it. Afterwards applicant will personally submit the application for a visa at the Embassy, along with other supporting documents, photo and a valid passport.
After completing an electronic visa application the applicant has the opportunity to book an appointment for submission of the application to the Embassy. Date and time of appointment will be confirmed subsequently by the Embassy.

The Embassy of Slovakia in Jakarta also reports that filling the application electronically, prior to the personal submission of application is not required.


Beginning December 5, 2011, the Embassy of the Czech Republic in Manila accepts visa applications of citizens of the Philippines for Schengen short term visas on behalf of the Slovak Republic.

An agreement between the Ministry of Foreign Affairs of the Slovak Republic and the Ministry of Foreign Affairs of the Czech Republic on the mutual representation by their diplomatic and consular missions in processing visas came into force on 4th December 2011.

According to the Agreement the Czech Republic represents Slovakia for the purpose of examining applications and issuing Schengen (short-term) visas on behalf of the Slovak Republic in the Republic of the Philippines.

This representation arrangement enables applicants to submit their visa applications in the Philippines at the Czech Embassy in Manila, instead of applying at the Embassy of the Slovak Republic in Jakarta, Indonesia.

Applications for long term visas are not subject to the representation arrangement and the applicants should continue to submit their visa applications at the Embassy of the Slovak Republic in Jakarta.
