Citizenship and Passport

Passport of the Slovak Republic can be issued to Slovak citizen only.  

To renew a still valid Slovak passport, please contact the Consular Section of the Embassy to set an appointment, preferebly least 3 months before the expiration date. 

If you do not have a valid Slovak passport or valid Slovak ID card, you will need to apply for the Certificate of Slovak Citizenship (Osvedčenie o štátnom občianstve) first. Only after obtaining the Certificate new passport can be issued. Children bellow age of 15 does not need to apply for the Certificate, if the child already has a Slovak birth certificate (Rodný list) and a parent has a valid Slovak passport. 

 

List of documents for Application for the Certificate of Citizenship of the Slovak Republic

Compulsory documents:

  • application form filled in Slovak language (will be filled at the appointment with the help of the Embassy employee)
  • Valid foreign passport of applicant
  • Slovak Birth Certificate (if not issued in the past, foreign Birth Certificate* may be accepted) 
  • Slovak Marriage certificate (if not issued in the past, foreign Marriage Certificate* may be accepted) - in case applicant is married 
  • Divorce certificate/court decision or Marriage certificate with included confirmation of divorce* - in case the applicant is divorced 
  • Israeli certificate of change of name or surname - in case the applicant has changed his name and there is no conformity between Slovak documents and Israeli passport  

 

Supporting documents that can help to prove the eligibility for Slovak citizenship, such as:

  • Invalid Slovak passport, if applicant had one in the past
  • Birth, Marriage and Death Certificates of a parent/grandparent of Slovak origin*
  • Valid or invalid Slovak passports or ID cards of a parent/grandparent of Slovak origin
  • Other documents that can help to prove the citizenship or residence of an ancestor in Slovakia and a link between the Slovak ancestor and applicant.

OTHER: 

  • Cash (about 50.00 ILS per application, depending on current EUR/NIS exchange rate)
  • Names, Surnames, Dates and Places of birth of all four grandparents of applicant 

 

When you will have all documents mentioned above ready, please contact the Consular department to set up an appointment.

You are required to bring originals and photocopies of all documents. We will compare photocopies with originals, keep copies with us and return originals back to you.  

 

* BIRTH/MARRIAGE/DEATH/DIVORCE certificates issued by the Israeli Authorities MUST BE: 

1. APPOSTILLED at the Ministry of Foreign Affairs in Jerusalem (Info about Apostilles) and 

 

2. TRANSLATED to SLOVAK language. The Slovak Embassy may provide official translations of Israeli Birth/Marriage Certificates to Slovak language, IF AGREED IN ADVANCE.
